After executing $GRID_HOME/root.sh script, if OS upstart fails to reload the oracleohasd entry automatically. Please follow below steps to troubleshoot this problem.
Step 1: Deconfigure the clusterware using below script if you have already executed root.sh script.[[email protected]] $GRID_HOME/crs/install/roothas.pl -delete -force -verbose
Step 2: Create a file /etc/init/oracle-ohasd.conf with the following content inside:
# Oracle OHASD startup
start on runlevel  stop on runlevel [!35] respawn
exec /etc/init.d/init.ohasd run >/dev/null 2>&1 </dev/null
Step 3 : Perform following steps to upstart ohasd process[[email protected]] initctl start oracle-ohasd
[[email protected]] initctl list |grep oracle
[[email protected]] oracle-ohasd
Step 4 : Rerun root script:[[email protected]] $GRID_HOME/root.sh
NOTE: You should see that the Oracle High Availibity process is started and root.sh finishes successfully.
Step 5 : Continue with the Grid Infrastruture installation.
